The Mizell Senior Center is always looking for new programs,
services and events to meet the interest and needs of Palm Springs
area residents. We currently offer over 145 programs and must be
careful in selecting new ones as space and resources are at a
premium. In order for us to fairly consider each new idea, the
following outline is being provided for your use.
The following questions should be answered in writing as
completely as possible and submitted to Mizell Senior Center.

 | Name of proposed program. |

 | Description of the program. |
|
 | What are the measurable outcomes of the program (how will
we know if it’s successful)? |
|
 | Who will lead this program and what are their related
qualifications and experience? |
|
 | Please provide three references on this leader. |
|
 | Is there standard certification for leaders, if so, who
provides this certification? |
|
 | Does the proposed leader have this certification? |
|
 | What is the minimum and maximum number of people to be
served by the program? |
|
 | What is the need or interest for this program and how was
this determined? |
|
 | Ideal time and frequency for program to meet. |
|
 | Has this type of program been offered in the community or at
the Center before, if so, what are the details? |
|
 | Is this type of program currently available elsewhere in the
community, if so what are the details? |
|
 | What are the suggested fees and charges and the expenses of
the program (including staff, equipment and supplies)? |
|
 | What, if any, storage needs will the program have? |
|
 | Provide the names, addresses and phone numbers of at least
ten members of the Center who would participate in this
program |
|
 | Is there a national governing body for this type of program,
if so what is the name of the organization and do they have
insurance for local programs? |
